CENTRAL COAST CASE MANAGEMENT SERVICES This service is to help people living in our community who are:
Central Coast Case Management Services provides individual case management for people in the HACC target group who are at risk of inappropriate or premature placement into residential care. Client needs include a package of services, complex care needs and high need for ongoing monitoring and co-ordination of services. Often clients have needs for a safety net service as the complexity and fluctuating nature of their care needs contribute to their difficulties in gaining access to the full range of necessary services. Central Coast Case Management Services is the ComPack provider in the Gosford Local Area. ComPacks provide case management and may include domestic assistance, personal care, shopping etc. The service is for people requiring 2 or more community services on discharge from hospital and require ComPacks support to facilitate discharge from hospital. Assessed as having clinical needs capable of being jointly met in the community by a ComPack and a clinical team.
